If you are new to Northwest Renal Clinic, we want to make your first visit as smooth as possible. This page provides important information and forms to help you prepare before your appointment.

Before Your Appointment

Please arrive 15 minutes before your scheduled appointment to allow time for check-in and any additional paperwork.

At your first visit, you will be asked to complete and review:

• Consent for services

• Authorization to release health information

• Acknowledgment forms

• Notice of Privacy Practices

You will be provided a copy of these documents upon request.
